NHS England commissions a data service from the Health and Social Care Information Centre, called Care.Data. This programme extracts information from GP practices every month. The data obtained is used by researchers to identify patterns in disease and the most effective treatments. The information extracted may include a patient’s;

- Date of birth
- Postcode
- Gender
The clinical information extracted is limited to certain types of data and Care.Data does not have full access to your GP record. If you wish, you may opt out of this programme. Summary Care Records (SCR)
Your SCR is an electronic record, containing information about your medication, allergies and any adverse reactions you may have had to medication. This information is made available to the National Data “spine” which is accessible to health professionals across the NHS.
Having such information available to the out of hours service and hospitals services improves patient safety and enhances the treatment of patients in an emergency. You may opt out of this service if you wish. To do so, please complete a separate opt-out form.
